Job Summary:
Provides skilled physical therapy services to patients in a skilled nursing facility, ensuring optimal recovery and mobility. Floats between facilities, delivering high-quality care across multiple locations as needed.

Responsibilities:

• Evaluate patients' physical conditions and develop personalized treatment plans.
• Implement therapy interventions to improve functional abilities and reduce impairments.
• Document patient progress and communicate treatment outcomes to the healthcare team.
• Collaborate with nursing staff and physicians to adjust therapy plans accordingly.
• Maintain clinical documentation in compliance with regulatory standards.
• Adapt therapy techniques for different facility settings and patient needs.

Qualifications:

• Valid state license as a Physical Therapist.
• Proven experience delivering therapy in skilled nursing or similar settings.
• Strong understanding of mobility, neurological, and musculoskeletal rehabilitation.
• Excellent communication and documentation skills.
• Ability to work independently and as part of a multidisciplinary team.
• CPR certification preferred.
