Job Summary:
Provide comprehensive occupational therapy services to patients with cognitive and ADL physical functioning disorders. Develop and implement individualized treatment plans following regulatory and clinical standards to improve patient outcomes.

Responsibilities:

• Assess patients’ cognitive and ADL physical functioning through standardized evaluations.
• Develop and execute personalized treatment plans utilizing evidence-based interventions.
• Administer rehabilitative procedures including ADL retraining, therapeutic exercises, modalities, and assistive device use.
• Monitor patient progress, adjust treatments accordingly, and document outcomes in the electronic health record.
• Collaborate with healthcare team members, patients, families, and caregivers to ensure coordinated care.
• Maintain compliance with all regulatory documentation, including discharge summaries and treatment minutes.

Qualifications:

• Hold a valid state Occupational Therapist license.
• Possess at least 1 year of OT experience, preferably in LTC or rehab settings.
• Demonstrate certification from NBCOT and adherence to AOTA ethics and practice standards.
• Have experience supervising OT assistants, aides, or students.
• Be able to obtain necessary DME or supplies as required.
• Exhibit strong communication and documentation skills.
