Job Summary:

Experienced Physical Therapist responsible for evaluating and providing patient-centered rehabilitation services in an acute care setting. This role involves developing and implementing individualized treatment plans to facilitate patient recovery and functional improvement.

Responsibilities:

• Conduct initial assessments to determine patients' physical therapy needs through comprehensive evaluations.
• Develop and implement customized treatment plans aligned with medical prescriptions and professional standards.
• Select appropriate assessment tools, techniques, and assistive devices to optimize patient outcomes.
• Educate patients and their families about treatment protocols, progress, and home exercises for ongoing rehabilitation.
• Document all clinical activities accurately in medical records, ensuring compliance with department and accreditation standards.
• Collaborate with multidisciplinary teams to coordinate patient care and refer to other clinical services as needed.

Qualifications:

• Hold a valid NYS Physical Therapist license.
• Possess a minimum of 2 years of clinical experience in an acute care environment.
• Demonstrate proficiency in clinical decision-making and treatment modifications.
• Exhibit strong communication skills to educate and engage patients and families.
• Maintain current knowledge of professional standards and best practices in physical therapy.
• Ability to work effectively in a fast-paced, collaborative healthcare setting.
