Job Summary:
The Physical Therapist in this home health role provides outpatient therapy services to adult patients in their homes, focusing on improving mobility, strength, and functional ability through personalized treatment plans.

Responsibilities:

• Evaluate patients' physical conditions and develop tailored treatment programs.
• Deliver therapy interventions to promote recovery and independence.
• Document patient progress and update care plans regularly.
• Educate patients and caregivers on exercises, safety, and injury prevention.
• Collaborate with multidisciplinary teams to coordinate care and optimize outcomes.
• Maintain timely and accurate documentation in EMR systems and adhere to compliance standards.

Qualifications:

• Hold a valid physical therapy license in the state of practice.
• Have at least 1 year of recent adult home health therapy experience.
• Proficient in OASIS documentation and recent use within the last 6 months.
• Demonstrate excellent communication and organizational skills.
• Willing to travel up to 75 miles from the primary clinic.
• Reliable transportation, valid driver’s license, and agency-required liability insurance.
