Job Description
Job Summary:
Provides specialized assessment, treatment, and rehabilitation services for patients with communication, cognitive, or swallowing disorders. Develops and implements individualized care plans to improve patient outcomes in accordance with clinical standards and regulations.
Responsibilities:
- Evaluate patients' communication, cognitive, and swallowing abilities using standardized assessments and evidence-based practices.
- Develop, implement, and modify treatment plans based on patient progress and clinical evaluations.
- Document all services, assessments, and patient progress accurately within electronic health records.
- Communicate effectively with patients, families, caregivers, and interdisciplinary team members regarding treatment progress and recommendations.
- Provide counseling and guidance to patients and families on managing speech and language disorders.
- Assist in supervising and mentoring CFYs or student Speech-Language Pathologists to support professional development.
Qualifications:
- Hold an active State Licensure and CCC certification as a Speech-Language Pathologist.
- Possess a minimum of 1 year of experience in speech-language pathology; new graduates with Level II experience are acceptable.
- Prior experience working in LTC or rehab settings preferred.
- Demonstrate strong clinical skills in evaluating and treating communication and swallowing disorders.
- Able to obtain necessary DME or supplies in accordance with clinical and regulatory standards.
- Comply with ASHA and state practice acts, ethics, and codes of conduct.
