Job Summary:
Seeking a dedicated Physical Therapist to provide comprehensive home health services to adult and geriatric patients. This role involves assessing, documenting, and managing patients within their homes while ensuring compliance with all clinical and regulatory standards.

Responsibilities:

• Assess patient needs and develop individualized treatment plans tailored to adults and elderly populations.
• Perform therapy sessions focused on orthopedics and neurology within a home health setting.
• Document patient progress accurately and ensure timely submission of all required reports.
• Maintain full compliance with agency policies, certification requirements, and safety protocols.
• Adapt to floating responsibilities within scope of practice to meet patient care needs.
• Complete assessments and documentation using HCHB unit systems efficiently.

Qualifications:

• Possess a minimum of 1 year of clinical experience as a Physical Therapist.
• Hold a current BLS certification.
• Valid licensure for Georgia or a Compact state.
• Demonstrate proficiency in orthopedic and neuro therapy techniques.
• Ability to work independently in a home environment with minimal supervision.
• Authorized to work in the U.S. without restrictions.
