Job Details

Job Summary:
Provide outpatient physical therapy services to adult and geriatric patients in a home health setting, ensuring individualized treatment plans to promote mobility, function, and recovery. Deliver care within a community-based environment while maintaining compliance with all clinical standards.

Responsibilities:

• Administer comprehensive assessments and develop personalized treatment plans for patients.
• Implement therapeutic interventions to improve functional ability and reduce pain.
• Document patient progress and adjust treatment plans accordingly.
• Collaborate with family members and caregivers to facilitate therapy continuity at home.
• Adhere to all regulatory and safety standards, including BLS certification.
• Travel within service counties and work within scope of practice, including floating as needed.

Qualifications:

• Hold a valid Physical Therapist license in the state of practice.
• Possess at least one year of outpatient or home health experience.
• Demonstrate proficiency with HCHB and other relevant electronic medical records.
• Be willing to travel within assigned counties with reliable transportation.
• Maintain current BLS certification.
• Capable of managing a patient ratio of 1:5 independently.
