Job Summary:
The Physical Therapist is responsible for evaluating, developing, and implementing treatment plans for adult and geriatric patients in a home health setting. This role involves providing skilled therapy to improve mobility, function, and quality of life, while ensuring compliance with all regulatory standards.

Responsibilities:

• Assess patient condition and develop individualized treatment plans based on clinical evaluations.
• Administer therapeutic interventions, including orthopedic and neurological therapies.
• Document patient progress, treatment outcomes, and maintain accurate records in Home Care Home Base (HCHB).
• Collaborate with healthcare team members to coordinate care and optimize patient outcomes.
• Ensure full compliance with licensing, certification, and facility policies.
• Adhere to infection control standards and safety protocols during all patient interactions.

Qualifications:

• Hold a valid Georgia license or applicable compact license with eligibility confirmed through licensing.allied@aya.
• Possess BLS certification.
• Minimum of 1 year of recent home health Physical Therapy experience; 1st timers accepted with screening.
• Demonstrate proficiency in assessments, documentation, and orthopedic and neurology therapies.
• Ability to work independently within scope of practice, floating as needed within the facility.
