Job Summary:
Provide specialized physical therapy services to adult and geriatric patients in a home health setting. Evaluate, develop, and implement treatment plans to improve patient mobility and recovery from conditions such as joint replacements, COPD, CHF, and T2DM.

Responsibilities:

• Assess patient needs and develop individualized care plans based on clinical evaluations.
• Deliver physical therapy interventions aligned with patient goals and clinical guidelines.
• Document patient progress, treatment provided, and discharge plans accurately.
• Collaborate with healthcare team members to ensure cohesive patient care delivery.
• Adhere to facility protocols, safety procedures, and compliance requirements.
• Maintain timely communication with supervisors regarding patient status and treatment updates.

Qualifications:

• Hold a valid GA or Compact state license as a Physical Therapist.
• Possess a minimum of 3 years of professional experience in home health or outpatient rehab.
• Demonstrate proficiency with HCHB or similar patient management systems.
• BLS certification is required.
• Experience with adult and geriatric patient populations.
• Ability to work independently within scope of practice and float as needed.
