Job Summary:
Provide patient-centered rehabilitation services as a Physical Therapy Assistant in a skilled nursing facility. Under the supervision of a licensed Physical Therapist, ensure residents receive effective therapy to enhance mobility and overall well-being.

Responsibilities:

• Implement prescribed treatment plans under the direction of a licensed Physical Therapist.
• Assist residents with exercises and therapeutic activities aimed at improving mobility and functional independence.
• Monitor and document patients’ progress and report changes to supervising therapists.
• Maintain a clean and safe therapy environment adhering to facility protocols.

Qualifications:

• Possess an active Physical Therapy Assistant license or registration.
• Have prior experience working in a skilled nursing or long-term care setting.
• Demonstrate strong communication and patient care skills.
• Ability to work effectively as part of a multidisciplinary team.
