Job Summary:
Provide comprehensive nursing care to residents in a long-term care or skilled nursing facility setting. Deliver high-quality, patient-centered services while ensuring adherence to regulatory standards and safety protocols.

Responsibilities:

• Assess residents' health status and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient progress, reporting changes to the healthcare team.
• Collaborate with interdisciplinary team members to ensure holistic patient care.
• Educate residents and their families on health management and post-discharge instructions.
• Maintain compliance with facility policies and state/federal regulations.

Qualifications:

• Hold an active and valid Registered Nurse license.
• Possess recent experience in LTC/SNF environments.
• Demonstrate proficiency in resident assessment and care planning.
• Show ability to handle multiple residents with compassion and professionalism.
• Be fully vaccinated against COVID-19 or have approved exemption documentation.
• Exhibit excellent communication and documentation skills.
