Job Summary:
Provide comprehensive nursing care to patients in a Long-Term Care or Skilled Nursing Facility setting. Ensure high-quality, patient-centered care while complying with facility policies and healthcare regulations.

Responsibilities:

• Deliver direct patient care by monitoring vital signs, administering medications, and managing treatments.
• Assess patient conditions and develop individualized care plans in collaboration with the healthcare team.
• Document patient progress accurately in electronic health records.
• Educate patients and families on health management and discharge instructions.
• Adhere to infection control protocols, including COVID-19 safety procedures.
• Maintain compliance with state and federal regulations concerning healthcare practices.

Qualifications:

• Valid and current Registered Nurse license.
• Minimum of one year experience in LTC/SNF settings preferred.
• Ability to comply with facility and safety policies, including COVID-19 vaccination requirements.
• Strong assessment and communication skills.
• Proficient in Electronic Health Record (EHR) systems.
• Must present a valid government-issued ID and be able to wear a photo badge at all times.
