Job Summary:
Provide skilled nursing care in a Long-Term Care/SNF setting, supporting residents with their daily healthcare needs. Ensure compliance with facility policies and deliver compassionate, patient-centered services.

Responsibilities:

• Administer medications and treatments accurately as prescribed by physicians.
• Monitor residents’ health status, recording vital signs and updates.
• Assist with activities of daily living, including hygiene, mobility, and nutrition.
• Follow safety procedures and maintain a sterile environment.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Document all patient interactions and care activities thoroughly and timely.

Qualifications:

• Valid LVN or LPN license in the state.
• Recent experience in LTC or SNF settings preferred.
• Ability to work efficiently in a fast-paced environment.
• Strong interpersonal and communication skills.
• Must have completed the COVID-19 vaccination or have an approved exemption.
• Certification in CPR and basic life support (BLS).
