Job Summary:
The Physical Therapist is responsible for evaluating, developing, and implementing individualized treatment plans to restore function and improve patient mobility. They work with patients recovering from illness or injury, ensuring optimal results within their capabilities. The role involves collaboration with care teams to promote patient well-being and independence.

Responsibilities:

• Assess patients’ conditions through initial evaluations and ongoing observations.
• Develop and execute tailored physical therapy programs to meet patient needs.
• Monitor and document patient progress, adjusting treatments as necessary.
• Educate patients and caregivers on exercises, mobility techniques, and safety procedures.
• Supervise Physical Therapy Assistants and coordinate care activities.
• Communicate patient status and treatment outcomes to the clinical team and report OASIS scores within established timelines.

Qualifications:

• Possess a valid state license as a Physical Therapist.
• Have at least one year of Home Health experience.
• Demonstrate strong clinical assessment and treatment planning skills.
• Exhibit excellent communication and teamwork abilities.
• Be able to perform detailed documentation and reporting of patient care.
• Show understanding of clinical standards and compliance requirements.
