Job Summary:
Provides comprehensive physical therapy services to patients with physical functioning disorders and wound care needs. Develops and implements individualized treatment plans to improve patient outcomes and promote functional independence.

Responsibilities:

• Assess patients’ physical functioning and establish appropriate treatment goals.
• Utilize standardized assessments and evidence-based practices to guide clinical interventions.
• Administer procedures such as manual techniques, therapeutic exercises, modalities, wound care, and use of supportive devices.
• Monitor patient progress, modify treatments as needed, and document outcomes in the clinical record.
• Educate patients, families, and caregivers on treatment plans, progress, and home exercises.
• Communicate regularly with the healthcare team regarding patient status and treatment adjustments.

Qualifications:

• Hold a valid Massachusetts State License as a Physical Therapist.
• Possess at least 1 year of clinical PT experience, preferably in LTC or rehab settings.
• Have the ability to supervise and support PT assistants, aides, or students.
• Demonstrate familiarity with all regulatory and clinical documentation requirements.
• New graduates with Level II experience are welcome to apply.
• Strong communication and teamwork skills.
