Job Summary:

The Physical Therapist assesses and implements treatment plans to enhance residents' functional ability and independence. Collaborating with residents and healthcare teams, the role focuses on delivering resident-centered therapy services to promote optimal outcomes.

Responsibilities:

• Perform evaluative and therapeutic procedures under physician or healthcare provider orders, adhering to clinical protocols.
• Develop individualized plans of care with clear goals, including therapy modalities, frequency, and duration.
• Consult with residents, caregivers, and the care team to optimize therapeutic strategies and discharge planning.
• Monitor and document treatment progress, adjusting plans as needed to meet established functional goals.
• Educate residents and caregivers on assistive devices, safety, and exercises, documenting teaching outcomes.
• Supervise and mentor assistant-level staff, ensuring compliance with professional standards and state mandates.

Qualifications:

• Hold an active Physical Therapist license in the applicable state.
• Possess a Bachelor's degree in Physical Therapy; a Master's or Doctorate is preferred.
• Demonstrate excellent communication, documentation, and interpersonal skills.
• Show experience or familiarity with long-term care or skilled nursing environments preferred.
• Maintain Basic Life Support (BLS) certification, if available.
