Job Summary:
Evaluate and develop comprehensive treatment plans for patients with physical functioning disorders and wounds. Provide rehabilitative therapies aimed at improving patient mobility, strength, and overall functional independence while adhering to regulatory standards.

Responsibilities:

• Assess patients’ physical conditions using standardized tools and evidence-based practices.
• Design and implement individualized treatment plans including manual therapy, therapeutic exercises, modalities, wound care, and assistance with supportive devices.
• Reassess patient progress regularly and modify treatments based on clinical outcomes and patient response.
• Educate patients, families, and caregivers on rehabilitation strategies and home exercise programs.
• Document all clinical interventions, assessments, and patient progress accurately in electronic health records.
• Collaborate with the healthcare team to coordinate care and participate in case management meetings.

Qualifications:

• Hold a Massachusetts Physical Therapist license.
• Possess at least 1 year of PT experience, including rehabilitation or long-term care settings.
• Demonstrate proficiency with clinical assessments, therapeutic procedures, and documentation standards.
• Able to supervise and support physical therapist assistants, aides, and students.
• Adhere to the American Physical Therapy Association (APTA) guidelines and state regulations.
• New graduates with Level II experience are welcome to apply.
