Job Summary:

Provide specialized physical therapy services to residents, aiming to enhance their functional abilities and independence in daily activities. Collaborate with healthcare team to develop and implement effective treatment plans.

Responsibilities:

• Evaluate residents’ physical conditions and develop individualized treatment plans based on department protocols.
• Perform evaluative and therapeutic procedures under physician’s orders, observing safety precautions.
• Monitor and adjust treatment programs to ensure progress toward established functional goals.
• Document all clinical interactions comprehensively in the Electronic Medical Record, ensuring clarity and accessibility.
• Educate residents and caregivers on therapeutic exercises, assistive device use, and care strategies.
• Collaborate with the healthcare team, including Physical Therapist Assistants, to coordinate resident care.

Qualifications:

• Hold an active license as a Physical Therapist in the applicable state.
• Possess a Bachelor’s degree in Physical Therapy; Master’s or Doctorate preferred.
• Have prior experience in a long-term care or skilled nursing facility preferred.
• Maintain Basic Life Support (BLS) certification (preferred).
• Strong communication, interpersonal, and documentation skills.
• Ability to lift up to 50 pounds and perform physical tasks required by the role.
