Job Summary:
Seeking an experienced Registered Nurse (RN) to provide home health care services in Carroll County, with support for Baltimore County as needed. The role involves delivering quality patient care within a scheduled daytime shift, maintaining productivity standards.

Responsibilities:

• Administer comprehensive home health nursing care to geriatric patients in accordance with care plans.
• Document patient visits and clinical information accurately using Oasis documentation standards.
• Collaborate with healthcare teams to ensure patient safety and effective care delivery.
• Maintain compliance with all licensing, certification, and travel requirements.
• Participate in on-call duties if required to support patient needs outside scheduled hours.
• Monitor patient progress and update care plans accordingly, ensuring quality and continuity of care.

Qualifications:

• Possess 2-3 years of home care nursing experience.
• Demonstrate proficiency with Oasis documentation processes.
• Maintain BLS certification and a valid driver’s license.
• Have a reliable vehicle for travel to patient homes.
• Licensed as an RN in Maryland (MD/Compact license).
• Ability to work Monday through Friday, 8:00 AM – 4:30 PM.
