Job Summary:

Seeking a dedicated Registered Nurse to provide high-quality home health care services. The nurse will support patients in their homes, ensuring clinical needs are met with compassion and professionalism.

Responsibilities:

• Administer skilled nursing care to patients in a home health setting according to physician orders.
• Document patient assessments, interventions, and progress accurately in electronic health records.
• Educate patients and families on disease management, medication administration, and health maintenance.
• Collaborate with physicians and interdisciplinary team members to develop and modify care plans.
• Maintain compliance with all state and federal regulations governing home health care.
• Adhere to infection control procedures and safety standards during patient visits.

Qualifications:

• Hold a valid and active state nursing license.
• Possess at least 1 year of home health nursing experience.
• Demonstrate proficiency with HCHB or similar home care Electronic Medical Records systems.
• Exhibit strong clinical assessment skills and the ability to work independently.
• Have reliable transportation and valid driver’s license.
• Be able to pass a background check and drug screening, including nicotine testing.
