Job Summary:
Seeking a dedicated Registered Nurse to provide quality home health care services in Baltimore County. The RN will assess, plan, and deliver patient-centered care to geriatric patients, ensuring compliance with documentation and safety standards.

Responsibilities:

• Conduct daily visits to home health patients, averaging six visits per day.
• Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
• Document care activities and patient progress accurately using Oasis documentation standards.
• Adhere to shift schedules from 8:00 AM to 4:30 PM, maintaining productivity expectations of 30 points weekly.
• Participate in on-call rotations as required to provide urgent patient support.
• Ensure compliance with travel regulations and maintain reliable transportation to patient locations.

Qualifications:

• Possess 2-3 years of home care nursing experience.
• Demonstrate proficiency in Oasis documentation.
• Hold a valid BLS certification and a current MD/compact RN license.
• Maintain a valid driver’s license and reliable vehicle.
• Must be able to work a daytime shift (8:00 AM to 4:30 PM).
