Job Summary:
Seeking a dedicated Home Health Registered Nurse to provide quality patient care in a hospital, SNF, or outpatient setting. This role involves managing diverse cases discharged from hospitals, MD offices, and SNFs, ensuring compliance with all regulations and protocols.

Responsibilities:

• Assess patients' health conditions and develop individualized care plans.
• Provide skilled nursing services, including wound care, medication management, and patient education.
• Document clinical assessments, interventions, and patient progress accurately in electronic health records.
• Collaborate with physicians, caregivers, and other healthcare team members to coordinate care.
• Maintain compliance with agency policies, state regulations, and safety standards.

Qualifications:

• Hold an active Maryland or Compact RN license.
• Demonstrate minimum 1 year of home care experience.
• Possess current certification in BLS.
• Be nicotine-free and pass a drug screen.
• Willing to float within scope of practice and hospital settings as needed.
• Provide two supervisor references.
