Job Summary:
Assess, plan, and implement personalized rehabilitation programs for patients with physical functioning disorders or wound care needs. Utilize evidence-based practices and standardized assessments to improve patient outcomes and ensure regulatory compliance.

Responsibilities:

• Evaluate patients’ physical conditions and develop tailored treatment plans aligned with clinical standards.
• Administer procedures such as manual therapy, therapeutic exercises, modalities, wound care, and assistive device management.
• Monitor patient progress regularly, adjusting treatments and documenting outcomes in compliance with regulations.
• Educate patients, families, and caregivers on treatment plans, exercises, and injury prevention strategies.
• Communicate effectively with the healthcare team regarding patient progress, barriers, and discharge planning.
• Support and supervise physical therapist assistants, rehabilitation aides, and students as needed.

Qualifications:

• Hold a valid State License to practice as a Physical Therapist in Oklahoma.
• Possess at least 1 year of physical therapist experience; new graduates with Level II experience are acceptable.
• Demonstrate experience in long-term care (LTC) or rehabilitation settings.
• Have a thorough understanding of clinical documentation, regulatory requirements, and treatment techniques.
• Maintain adherence to APTA guidelines, ethical standards, and practice acts.
