Job Summary:
Provides skilled physical therapy services to patients with physical functioning disorders and/or wounds, focusing on improving mobility, strength, and overall independence. Develops individualized treatment plans aligned with clinical standards and regulatory guidelines to facilitate patient recovery and well-being.

Responsibilities:

• Screen, evaluate, and develop personalized care plans for patients with physical disabilities or wound care needs.
• Administer therapeutic procedures including manual techniques, ambulation support, therapeutic exercises, modalities, and wound management.
• Monitor patient progress, adjust treatments as needed, and document outcomes accurately in compliance with regulations.
• Educate patients, families, and caregivers on treatment goals, techniques, and injury management.
• Communicate patient progress and collaborate with healthcare team members to optimize recovery plans.
• Supervise and support physical therapist assistants, rehabilitation aides, and students.

Qualifications:

• Hold a valid Oklahoma State License as a Physical Therapist.
• Possess at least 1 year of PT experience, ideally in LTC or rehab settings.
• Demonstrate proficiency in assessed clinical procedures and documentation standards.
• Be open to recent graduates with Level II experience, not just observational roles.
• Adhere to APTA guidelines and state practice acts, maintaining high ethical standards.
• Exhibit excellent communication skills and a compassionate approach to patient care.
