Job Summary:
Provide specialized rehabilitative care to patients with physical functioning disorders and wound care needs. Develop individualized treatment plans to improve patient outcomes while ensuring compliance with all regulatory and clinical standards.

Responsibilities:

• Assess patient conditions using standardized tools and evidence-based practices to create effective treatment strategies.
• Administer therapeutic procedures including manual techniques, ambulation, exercises, modalities, supportive devices, and wound care.
• Monitor and evaluate patient progress, adjusting treatment plans accordingly and documenting outcomes thoroughly.
• Communicate with patients, their families, caregivers, and healthcare team members regarding treatment plans and progress.
• Maintain accurate and complete documentation in electronic health records, including discharge summaries and treatment minutes.
• Participate in team meetings, training sessions, and clinical conferences to stay current with best practices.

Qualifications:

• Hold a valid state license to practice as a Physical Therapist in Oklahoma.
• Possess at least 1 year of physical therapy experience, preferably in LTC or rehab settings.
• New graduates with Level II experience are eligible.
• Demonstrate knowledge of clinical assessment, manual techniques, and wound care procedures.
• Ability to supervise and support Physical Therapist Assistants, aides, or students.
• Adhere to APTA guidelines, state regulations, and ethical standards.
