Job Summary:

Seeking a dedicated Physical Therapist to provide home health services, including initial evaluations and follow-ups, within a home care setting. The role involves travel to patient residences, documentation, and coordination with care teams to ensure quality patient outcomes.

Responsibilities:

• Perform initial assessments and follow-up visits for adult patients in their homes.
• Document all visits accurately within established timeframes and maintain compliance with agency standards.
• Travel up to 75 miles from the home clinic to patient locations, ensuring timely and efficient care delivery.
• Collaborate with care teams during periodic office meetings and potentially float to nearby clinics as needed.
• Complete documentation within 24 hours of patient visits to support billing and care continuity.
• Adhere to all safety and documentation protocols, including maintaining current driver’s license and liability insurance.

Qualifications:

• Hold a valid state license as a Physical Therapist in good standing.
• Minimum of 1 year of recent adult home health experience.
• Demonstrate proficiency in OASIS, HomeCare HomeBase EMR, and other relevant documentation systems.
• Possess reliable transportation, auto insurance, and a current driver’s license.
• Ability to travel up to 75 miles and accept a flexible schedule, including on-call rotations if applicable.
• Certified in CPR and BLS.
