Job Summary:

The Registered Nurse in Case Management within a hospital setting coordinates patient care, ensuring appropriate utilization of resources and compliance with regulatory standards. This role focuses on facilitating patient care plans and optimizing recovery outcomes.

Responsibilities:

• Assess patients' clinical needs and collaborate with healthcare teams to develop appropriate care plans.
• Review medical records to ensure adherence to utilization review and levels of care protocols.
• Communicate effectively with physicians, patients, families, and care teams to promote coordinated care.
• Monitor patient's progress and adjust care plans as needed to meet regulatory and clinical standards.
• Ensure compliance with healthcare laws, regulations, and accreditation standards affecting case management.
• Document all case management activities accurately within electronic health records.

Qualifications:

• Valid CA Registered Nurse license with Nursys verification uploaded within the last 2 weeks.
• Minimum of 2 years recent relevant experience in hospital case management.
• Strong knowledge of healthcare reimbursement systems, including HMO, PPO, PPS, CMS, and value-based models.
• Familiarity with laws and regulations such as CMS, Title 22, CHA, CDPH, and TJC.
• Excellent communication, negotiation, and time management skills.
• Proficiency in electronic health records and intermediate computer skills.
