Job Summary:
Coordinates utilization management, discharge planning, resource coordination, and post-acute care referrals for acute patient populations. Focuses on providing integrated care to reduce unnecessary admissions and readmissions, ensuring optimal patient outcomes and efficient utilization of healthcare resources.

Responsibilities:

• Coordinate utilization management, discharge planning, and care facilitation for high-risk patients.
• Oversee management of acute patient populations across the care continuum to prevent unnecessary hospitalizations.
• Provide discharge planning and intervention for patients presenting to the Emergency Department.
• Collaborate with interdisciplinary teams to promote patient wellness and improve care outcomes.
• Ensure compliance with documentation, certification, and licensure requirements for all patients.
• Maintain accurate records of patient care activities and ensure timely completion of modules and onboarding processes.

Qualifications:

• CA Registered Nurse license (Nursys verification within the last 2 weeks).
• Minimum of 2 years of relevant experience in case management or hospital setting.
• Current BLS certification through AHA or ARC.
• Ability to coordinate care effectively within a multidisciplinary team.
• Strong understanding of healthcare regulations and compliance standards.
• Excellent communication and documentation skills.
