Job Summary:
The Nurse Case Manager provides comprehensive patient care coordination by collaborating with interdisciplinary teams, patients, and families to optimize health outcomes. They oversee the continuum of care from pre-admission through post-discharge, ensuring efficient and appropriate service delivery.

Responsibilities:

• Coordinate patient care activities across multiple disciplines to ensure timely and appropriate interventions.
• Develop, implement, and revise individual care plans in collaboration with physicians and healthcare teams.
• Monitor diagnostic tests, treatments, procedures, and resource utilization to ensure alignment with clinical needs.
• Liaise with physicians to discuss discharge planning, estimated length of stay, and resource use.
• Manage complex patient populations, including frequent ED utilizers, chronic pain, substance abuse, and homeless patients.
• Facilitate patient transfers to tertiary centers, ensuring all necessary information is transferred for continued care.

Qualifications:

• Current Registered Nurse (RN) licensure in the state of practice.
• Minimum of two years recent experience in care management or case management field.
• Valid Basic Life Support (BLS) certification from an approved provider.
• Experience with inpatient case management and high patient caseloads preferred.
• Ability to handle complex cases with minimal training, including strike force experience with KP or HCA CA.
• Strong communication and interdisciplinary collaboration skills.
