Job Summary:
Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure the delivery of compassionate, quality patient care while adhering to facility policies and regulatory standards.

Responsibilities:

• Assess patients' health conditions and observe changes in their status.
• Administer medications and treatments accurately and safely.
• Document patient care and progress in accordance with regulatory requirements.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Educate patients and their families on health management and treatment options.
• Maintain compliance with infection control protocols and safety guidelines.

Qualifications:

• Valid LVN/LPN license in the state of practice.
• Proven experience in LTC/SNF or similar long-term care environments.
• Strong assessment and clinical skills.
• Ability to communicate effectively with patients, families, and team members.
• Knowledge of state and federal healthcare regulations.
• Basic life support (BLS) certification.
