Job Summary:
Provides skilled physical therapy services to patients in a skilled nursing facility to improve mobility, strength, and overall functional ability. Collaborates with interdisciplinary teams to develop and implement personalized treatment plans.

Responsibilities:

• Evaluate patients’ physical functions and develop customized treatment programs.
• Implement therapeutic interventions aimed at improving mobility, balance, and strength.
• Monitor patient progress and adjust treatment plans accordingly.
• Educate patients and caregivers on exercises and injury prevention strategies.
• Document patient assessments, interventions, and progress accurately.
• Comply with all healthcare regulations, safety standards, and facility policies.

Qualifications:

• Valid physical therapy license in the state of practice.
• Proven experience working in SNF or post-acute care settings.
• Strong knowledge of musculoskeletal, neurological, and orthopedic conditions.
• Excellent communication and patient interaction skills.
• Ability to collaboratively work within an interdisciplinary healthcare team.
