Job Summary:
Provide specialized physical therapy services to patients in a skilled nursing facility setting. Assess patient needs, develop individualized treatment plans, and deliver therapeutic interventions to promote recovery and mobility.

Responsibilities:

• Evaluate patients' physical conditions and mobility levels to determine appropriate therapy goals.
• Develop and implement tailored treatment plans based on patient assessments.
• Administer therapeutic exercises, manual therapy, and other modalities to improve patient function.
• Monitor patient progress and adjust treatment plans accordingly to optimize outcomes.
• Document patient care activities, progress, and response to therapy in electronic health records.
• Collaborate with healthcare team members to coordinate comprehensive patient care.

Qualifications:

• Hold a valid and current Physical Therapist license in the state of practice.
• Possess a Degree in Physical Therapy (DPT) from an accredited program.
• Demonstrate experience working in SNF or geriatric care settings preferred.
• Exhibit strong clinical assessment and treatment planning skills.
• Maintain excellent communication and teamwork abilities.
• Be able to perform physical tasks necessary for therapy interventions.
