Job Summary:
Provide direct patient care to residents in long-term care or skilled nursing facility settings, supporting their daily activities and ensuring comfort and safety. This role involves collaborating with the healthcare team to meet patients' needs effectively.

Responsibilities:

• Assist residents with activities of daily living, including bathing, dressing, and mobility.
• Monitor, record, and report changes in residents’ condition to the nursing staff.
• Follow care plans accurately and ensure compliance with healthcare regulations.
• Maintain a clean and safe environment for residents at all times.
• Support medication administration under supervision and assist with basic medical tasks as instructed.

Qualifications:

• Valid CNA certification and active state licensure.
• Proven experience in LTC/SNF or similar healthcare settings.
• Demonstrate strong communication and compassion skills.
• Ability to follow detailed care plans and adhere to safety protocols.
• Maintain current CPR/BLS certification.
