Job Summary:
Provides specialized nursing care to patients in long-term acute care (LTAC) settings, focusing on complex medical conditions such as neuro and cardiac needs. Delivers patient-centered support and manages care plans in accordance with facility protocols.

Responsibilities:

• Assess and monitor the health status of LTAC patients, including neuro and cardiac cases.
• Administer medications, treatments, and interventions as prescribed by physicians.
• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
• Document patient progress, treatments, and adverse reactions accurately in EMR systems.
• Respond promptly to patient needs and provide emergency interventions when necessary.
• Support patient recovery and educate patients and families about care requirements post-discharge.

Qualifications:

• Hold a valid Registered Nurse (RN) license.
• Possess a minimum of 2 years of nursing experience with at least 1 year in MS/TELE or LTAC settings.
• Demonstrate proficiency with EPIC EMR software.
• Show experience with neuro and cardiac patient care preferred.
• Be able to work night shifts (4x12-hour shifts) and participate in rotating call responsibilities.
• Meet travel compliance standards with at least 100 miles from participating Prisma facilities.
