Job Summary:
Seeking a qualified California RN for hospital case management during the flu season, requiring acute care experience and flu vaccination compliance. The role involves coordinating patient care, ensuring regulatory adherence, and utilizing clinical judgment to support patient outcomes.

Responsibilities:

• Manage hospital cases by assessing patient needs and developing appropriate care plans.
• Collaborate with healthcare teams to coordinate discharge planning and resource allocation.
• Ensure compliance with vaccination mandates, specifically flu vaccine requirements.
• Document clinical activities and patient progress accurately within the electronic health record system, preferably EPIC.
• Adhere to state licensure requirements, maintaining active California RN license at all times.
• Follow organizational policies regarding scheduling, RTO requests, and background reviews.

Qualifications:

• Hold a valid California RN license in good standing.
• Possess at least 3 years of acute care case management experience.
• Demonstrate proficiency with EPIC or similar electronic health record systems.
• Be vaccinated against the flu, with no exemptions permitted.
• Ensure ability to work daytime shifts 8:00 am – 5:00 pm, with flexibility for assignment needs.
• Maintain current BLS certification.
