Job Summary:
Seeking an experienced acute care case manager registered nurse to coordinate patient care within a hospital setting. This role involves collaborating with healthcare teams to ensure comprehensive patient management and discharge planning.

Responsibilities:

• Evaluate patient needs and develop individualized care plans to optimize health outcomes.
• Coordinate with multidisciplinary teams to facilitate seamless patient care transitions.
• Document clinical assessments, interventions, and patient progress accurately in the electronic health record system.
• Ensure compliance with hospital protocols, state regulations, and certification requirements.
• Educate patients and their families about treatment plans and post-discharge care instructions.
• Promote patient safety and quality standards through diligent monitoring and assessment.

Qualifications:

• Hold a valid California RN license active at the time of submission.
• Possess at least 3 years of acute care case management experience.
• Maintain current BLS and flu vaccination certifications.
• Demonstrate proficiency with Epic electronic health record systems, highly preferred.
• Exhibit strong clinical assessment, communication, and organizational skills.
• Be flexible to work day shifts and adapt to hospital needs.
