Job Summary:
We are seeking a dedicated Physical Therapist to provide exceptional rehabilitation services within a skilled nursing facility setting. This role involves evaluating patient needs and developing effective treatment plans to improve mobility and function.

Responsibilities:

• Assess patient condition and establish personalized treatment goals.
• Implement therapeutic interventions to promote mobility, strength, and overall functional independence.
• Monitor patient progress and adjust treatment plans accordingly.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Educate patients and families on strategies to maintain improvements and prevent future issues.
• Document patient assessments, treatment plans, and progress reports accurately.

Qualifications:

• Valid state licensure as a Physical Therapist.
• Proven experience working with adult and geriatric populations in a skilled nursing environment.
• Strong knowledge of rehabilitative techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• CPR certification preferred.
