Job Summary:
Provides specialized physical therapy services to patients with physical functioning disorders and/or wound care needs. Develops and implements individualized treatment plans to promote healing, mobility, and functional independence.

Responsibilities:

• Evaluate patients’ physical conditions using standardized assessments and clinical judgment.
• Develop and modify personalized treatment plans based on patient progress and clinical standards.
• Administer therapeutic procedures including manual techniques, ambulation, exercises, modalities, wound care, and use of assistive devices.
• Monitor and document patient progress, adjusting treatments accordingly, and record outcomes in compliance with regulatory requirements.
• Communicate effectively with patients, families, caregivers, and interdisciplinary team members regarding treatment plans and patient progress.
• Document all clinical encounters accurately in the electronic health records and participate in team meetings and reviews.

Qualifications:

• Hold a valid Massachusetts State License as a Physical Therapist.
• Possess at least 1 year of physical therapy experience, with LTC or rehabilitation setting preferred.
• Demonstrate proficiency in clinical assessment and intervention techniques.
• Ability to supervise and support PTA assistants, aides, or students.
• Adhere to APTA standards and state regulations, maintaining professional conduct and ethics.
