Job Summary:
An Occupational Therapist is sought to join a skilled rehab team providing therapy services in a Skilled Nursing Facility. The role involves conducting evaluations, providing daily treatments, and managing patient discharges to facilitate optimal recovery.

Responsibilities:

• Evaluate patients' functional abilities and develop personalized treatment plans.
• Administer daily therapeutic interventions in accordance with care plans.
• Document patient progress and adjust treatment strategies as needed.
• Collaborate with interdisciplinary team members to coordinate patient care.
• Ensure compliance with facility policies and professional standards.
• Manage patient discharges and provide appropriate recommendations for continued care.

Qualifications:

• Hold an active Occupational Therapist license in New Mexico or eligible for licensure.
• Possess prior SNF experience highly preferred.
• Demonstrate strong clinical assessment and treatment skills.
• Exhibit excellent communication and documentation abilities.
• Be able to commit to working in the facility to guarantee hours.
• Maintain professional appearance adhering to dress code standards.
