Job Summary:


Seeking an experienced Occupational Therapist to join a skilled nursing facility in Albuquerque, NM. The therapist will provide essential rehabilitation services to a high-volume patient population, ensuring quality care and discharge planning.



Responsibilities:

• Conduct comprehensive initial evaluations and develop personalized treatment plans.
  
• Administer daily therapeutic treatments to patients based on individualized care plans.
  
• Monitor patient progress and adjust therapy interventions accordingly.
  
• Document patient sessions accurately and maintain timely records in compliance with facility policies.
  
• Collaborate with the interdisciplinary team to facilitate patient discharge and ensure seamless care transition.
  
• Adhere to dress code policy and productivity standards, maintaining a 90% required productivity rate.
  
  
  
  Qualifications:
  
• Hold a valid Occupational Therapist license in the state of New Mexico.
  
• Demonstrate prior experience working in a skilled nursing facility (SNF).
  
• Possess strong assessment and treatment planning skills.
  
• Maintain excellent communication and documentation abilities.
  
• Be available to start within 2-3 weeks of offer acceptance.
  
• Attend in-house and follow facility-specific policies including dress code and productivity expectations.
