Job Summary:
Provide skilled physical therapy services in a hospital outpatient and inpatient setting at a Critical Access Hospital. Collaborate with healthcare teams to support patient recovery and rehabilitation.

Responsibilities:

• Evaluate patients’ physical conditions and develop personalized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and function.
• Document patient progress and adjust therapy plans accordingly.
• Collaborate with physicians, nurses, and other healthcare providers to coordinate care.
• Educate patients and families on exercises and wellness strategies.
• Ensure compliance with safety standards and maintain accurate documentation.

Qualifications:

• Hold a current and valid New Hampshire state license as a Physical Therapist.
• Possess at least 2 years of recent outpatient physical therapy experience.
• Demonstrate proficiency with physical therapy skills and procedures.
• Provide a signed supervisory reference from the past year.
• Maintain active BLS certification from the American Heart Association.
• Submit all required professional licenses, certifications, and proof of current credentials.
