Job Summary:
The Occupational Therapist will provide personalized therapy services to patients in a skilled nursing facility, focusing on improving independence and quality of life. You will collaborate with interdisciplinary teams to develop and implement effective treatment plans tailored to each patient’s needs.

Responsibilities:

• Conduct comprehensive assessments of patients’ physical, mental, and emotional health.
• Develop individualized treatment plans to enhance functional independence in activities of daily living (ADLs).
• Deliver direct therapy interventions, including therapeutic exercises, adaptive equipment training, and cognitive training.
• Collaborate with healthcare team members to coordinate patient care and ensure optimal outcomes.
• Educate patients and families on home safety, adaptive techniques, and the use of equipment.
• Document patient progress and adjust treatment plans to meet regulatory and facility standards.

Qualifications:

• Hold a degree in Occupational Therapy from an accredited program.
• Possess a valid Occupational Therapy license in CA.
• Have experience working in a skilled nursing or geriatric care setting preferred.
• Demonstrate strong knowledge of rehabilitation techniques, modalities, and equipment.
• Exhibit excellent communication, organizational, and interpersonal skills.
• Maintain a compassionate, patient-centered approach to care.
