Job Details

Job Summary:
Provides expert physical therapy services to patients with physical functioning disorders and/or wound care needs. Develops and implements individualized treatment plans to promote recovery and improve patient outcomes in compliance with regulatory standards.

Responsibilities:

• Assess patients' physical condition and establish personalized treatment plans based on standardized assessments and evidence-based practices.
• Administer therapeutic interventions, including manual techniques, ambulation support, therapeutic exercises, modalities, and wound care.
• Monitor and evaluate patient progress, adjusting treatments accordingly and documenting outcomes accurately in medical records.
• Educate patients, families, caregivers, and other healthcare providers on treatment protocols and discharge instructions.
• Communicate effectively with the healthcare team regarding patient progress, barriers, and treatment modifications.
• Supervise and support physical therapist assistants, rehabilitation aides, and students to ensure quality care delivery.

Qualifications:

• Hold an active Oklahoma state license to practice as a Physical Therapist.
• Possess at least 1 year of physical therapy experience, with LTC or rehab backgrounds preferred.
• Demonstrate proficiency in clinical assessment, treatment planning, and documentation processes.
• Exhibit strong communication skills and ability to work collaboratively within a healthcare team.
• Ability to support and supervise rehabilitation staff and obtain necessary DME or supplies per regulatory requirements.
