Job Summary:
Provide comprehensive physical therapy services to patients with physical functioning disorders and wound care needs. Develop and implement individualized treatment plans to improve patient outcomes in a regulated clinical environment.

Responsibilities:

• Evaluate patients’ physical conditions utilizing standardized assessments and evidence-based practices.
• Administer treatment procedures including manual techniques, therapeutic exercises, modalities, assistive devices, and wound care.
• Monitor patient progress, adjust treatments accordingly, and document outcomes in compliance with regulatory standards.
• Collaborate with patients, families, caregivers, and the healthcare team to ensure optimal care and discharge planning.
• Communicate effectively with supervisors and team members regarding patient status, treatment plans, and barriers to progress.
• Maintain accurate, thorough documentation of all services provided and participate in clinical meetings and trainings.

Qualifications:

• Hold a valid Oklahoma State Physical Therapist license.
• Possess at least 1 year of licensed PT experience, preferably in LTC or rehab settings.
• Demonstrate proficiency in clinical assessments and therapeutic interventions.
• Ability to supervise and support PTA, rehab aides, and students.
• Compliance with APTA standards, state practice acts, and professional ethics.
• Strong communication and documentation skills.
