Job Summary:

Provide skilled rehabilitative services to patients with cognitive and physical disorders under the supervision of an Occupational Therapist. Support patient recovery and functional independence through evidence-based interventions and documentation.

Responsibilities:

• Assist in implementing treatment plans designed by the Occupational Therapist, including ADL retraining, therapeutic activities, exercises, and assistive device use.
• Evaluate patient progress and communicate findings to the supervising Occupational Therapist to modify treatments as needed.
• Educate patients, families, caregivers, and other healthcare providers about treatment goals and strategies related to cognitive or physical impairments.
• Document all clinical interactions and treatment data accurately in electronic health records, adhering to regulatory standards.
• Participate in team meetings, care conferences, and relevant training sessions to ensure effective patient care.
• Support the procurement of necessary DME and supplies, aligning with clinical protocols and documentation requirements.

Qualifications:

• Minimum of 1 year of experience as an Occupational Therapy Assistant.
• Valid COTA license and active state licensure/registration.
• Prior experience in long-term care or rehabilitation settings.
• Strong communication skills to collaborate with multidisciplinary teams.
• Demonstrated ability to follow regulatory and clinical documentation standards.
