Job Summary:
Provide comprehensive occupational therapy services to patients with cognitive and ADL physical functioning disorders. Develop and implement individualized treatment plans to improve patient independence and quality of life, following all regulatory and clinical standards.

Responsibilities:

• Evaluate patients' cognitive and physical functioning to determine treatment needs.
• Develop and execute personalized care plans based on standardized assessments and evidence-based practices.
• Administer therapeutic activities, exercises, modalities, and assistive devices to facilitate patient rehabilitation.
• Monitor patient progress, adjust treatments, and document outcomes in compliance with regulations.
• Educate patients, families, and caregivers regarding treatment goals, interventions, and adaptive strategies.
• Collaborate effectively with healthcare team members to optimize patient care and ensure timely referrals.

Qualifications:

• Hold an active OT license in the applicable state.
• Possess 1+ years of occupational therapy experience, preferably in LTC or rehab settings.
• Demonstrate proficiency in clinical assessments, documentation, and treatment implementation.
• Be willing to consider new graduates with Level II clinical experience.
• Submit a current resume and skills checklist within one year.
• Provide 0-1 references upon submission.
