Job Summary:

Occupational Therapist providing skilled rehabilitation services to patients with cognitive and/or activities of daily living (ADL) physical functioning disorders. Evaluates patient needs, develops individualized plans of care, and delivers evidence-based interventions to improve functional independence. Documents outcomes and collaborates with the interdisciplinary team to support safe, effective care.

Responsibilities:

• Screen and evaluate patients to identify cognitive and ADL functional needs.
• Develop and implement individualized plans of care using standardized assessments and evidence-based practice.
• Deliver rehabilitative interventions including ADL retraining, therapeutic activities, therapeutic exercises, and therapeutic modalities.
• Select and train patients on supportive and assistive devices to promote participation and safety.
• Monitor progress, adjust treatment plans as needed, and document treatment outcomes in the electronic record.
• Provide patient and caregiver consultation and complete comprehensive discharge summaries per regulatory requirements.

Qualifications:

• Hold an active Occupational Therapist license.
• Demonstrate at least 1+ year of Occupational Therapy experience (new grads considered with Level II experience).
• Provide required skills checklist documentation within the past year.
• Possess LTC or rehab experience.
• Adhere to NBCOT standards and applicable state practice acts, ethics, and professional conduct.
• Maintain accurate records, including treatment minutes, and participate in required care meetings and trainings.
