Job Summary:
Seeking experienced Registered Nurses to provide compassionate and professional care in a skilled nursing facility with a mix of long-term and short-term residents. Candidates will deliver high-quality patient care while collaborating with a multidisciplinary team.

Responsibilities:

• Administer medications, including IVs, PICC lines, and mediports, following physician instructions and facility protocols.
• Perform wound care, Foley/catheter care, PEG tube feedings, ostomy, and tracheostomy care.
• Monitor and assess patient conditions, responding appropriately to changes in clinical status.
• Collaborate with interdisciplinary team members such as PT, OT, SLP, case managers, and social workers.
• Maintain accurate documentation of patient care and treatment plans in Point Click Care EMR system.
• Participate in team conferences, shift handoffs, and ongoing quality improvement initiatives.

Qualifications:

• Valid RN licensure in the state of practice.
• CPR certification required prior to first day of employment.
• Experience with IV therapy, PICC lines, mediports, and central lines preferred.
• Familiarity with SNF/LTC patient populations, including complex chronic conditions.
• Ability to work 12-hour shifts, every other weekend, and adhere to facility scheduling policies.
