Job Summary:
Provide skilled physical therapy services to patients with physical functioning disorders and wound care needs. Develop and implement individualized treatment plans to enhance mobility, manage wounds, and improve patient outcomes.

Responsibilities:

• Evaluate patients’ physical conditions using standardized assessments and establish appropriate care plans.
• Administer therapeutic interventions including manual techniques, ambulation, exercises, modalities, wound care, and assistive device training.
• Monitor and document patient progress, adjusting treatments as needed to optimize recovery outcomes.
• Educate patients, families, and caregivers on treatment procedures, self-care, and discharge instructions.
• Collaborate with healthcare team members to coordinate care and communicate patient progress.
• Maintain accurate documentation of treatments, patient records, and regulatory compliance within electronic health systems.

Qualifications:

• Hold a valid MA State Physical Therapist license.
• Possess at least 1 year of physical therapy experience, with LTC or rehab background preferred.
• Demonstrate proficiency in wound care, therapeutic exercises, and modalities.
• Ability to supervise and support PTA’s, aides, and students effectively.
• Adhere to APTA standards and state regulations regarding professional conduct and ethics.
• Capable of obtaining necessary DME or supplies according to clinical requirements.
