Job Summary:
Provide outpatient home health physical therapy services to adult patients, focusing on functional improvement and mobility. Deliver personalized patient care in accordance with physician orders and industry standards.

Responsibilities:

• Assess patients’ physical condition and develop customized treatment plans.
• Implement therapeutic exercises and interventions to improve mobility and function.
• Educate patients and caregivers on exercises, safety, and injury prevention.
• Document patient progress, treatment, and discharge summaries accurately in EMR.
• Comply with all travel and documentation protocols, including BLS and state licensing requirements.
• Collaborate with healthcare team members to optimize patient outcomes.

Qualifications:

• Hold a Maryland Physical Therapy License or be eligible to submit pending license.
• Possess at least 2 years of outpatient home health PT experience.
• Demonstrate proficiency with EMR systems.
• Maintain current BLS certification.
• Have prior experience in home health physical therapy settings.
• Be prepared to meet productivity expectations of 30 patients per week.
